• Lost Paycheck?

  State Employees:  Complete and submit this Lost Paycheck Form to State Payroll by either.

      3. In person: Admin Building, Rm 390 - Mon -Fri  9am-4pm.
      4. Mail  to State Payroll, Admin Building, Rm 390 Zip = 0751

• What happens if I forgot to pick up my paper advice statement?

  State Employees: Direct deposit advice statements are distributed by the enrollee's agency. If the statement is unclaimed, it will be held by the agency for thirty (30) days after which time the statement will be destroyed.
