Frequently Needed Forms

*All forms must be submitted digitally at Bio_Registration@stonybrook.edu, not brought to the Undergraduate Biology office*

Deadline for all Fall 2023 Registration forms will be Sunday, September 3rd, 2023 to ensure that the forms can be processed prior to the add/drop deadline. Any forms received after this date are not guaranteed to be processed before the add/drop deadline.

• Major/Minor Declaration forms can be submitted until Friday, September 1, 2023. The declaration will be effective as of Fall 2023 semester. The form can be found on SOLAR by going to student records and registration > enrollment > undergraduate major/minor form.

BIO/BCH Research Registration:

1 - Complete Student Reflection

2- Obtain Faculty Sponsor Signature
