Sample Non-Salaried Appointment Letter

Departments should use this template when inviting a visitor to a non-salaried appointment at Stony Brook University.

Instructions

Before issuing the appointment letter:

  • Replace all placeholder text with the appropriate information.
  • Include the Terms and Conditions for Non-Salaried Appointments with the appointment letter.
  • Obtain the visitor's signature acknowledging acceptance of both the appointment and the Terms and Conditions.
  • Retain a signed copy of the appointment letter and Terms and Conditions in the department's records.
