Departments sponsoring international students, scholars, and exchange visitors may
be required to submit supporting documentation as part of the immigration request
process. The forms and template letters below are designed to help departments meet
federal requirements and provide Visa and Immigration Services with the information
needed to process requests efficiently.
Please review the instructions on each page carefully and customize all templates
before use. If you have questions about which document is required for your situation,
contact Visa and Immigration Services for assistance.

Document an interview conducted to objectively assess the English language proficiency
of a prospective J-1 Research Scholar, Professor, or Short-Term Scholar when a standardized
test or institutional certification is not available.
Obtain certification from a student's home institution confirming eligibility for
participation in the J-1 Student Intern Program and verifying that the internship
supports the student's academic program.
Use this template to offer a temporary non-salaried appointment to a visiting scholar
or researcher, including required appointment language and acceptance provisions.
Certification for Exchange Visitors with Clinical or Medical Degrees (No Patient Contact)
Certify that a J-1 Exchange Visitor holding a clinical or medical degree will participate
exclusively in observation, consultation, teaching, or research and will not engage
in patient care.
If you're unsure which form or template applies to your visitor or have questions
about the documentation requirements, please contact Visa and Immigration Services before submitting your request. We're happy to help ensure your documentation is
complete and meets federal requirements.
