Requirements for Full-time Students
All students attending courses on Stony Brook campus must submit a completed Immunization / Meningitis Form, demonstrating compliance with New York State Health Laws. It is REQUIRED that this form be submitted to Student Health at least 3 weeks prior to the student’s enrollment and or orientation. If the immunization / Meningitis forms have not been received by this deadline a late fee may be incurred, and additional enrollment will be prohibited.
Required forms and instructions for submission can obtained here: Immunization/Meningitis-Health Forms
All full-time students must have health insurance coverage at all times without exception . Stony Brook University offers a health insurance plan for all domestic students that meet this requirement. This plan pays for most medically necessary bills, such as doctor visits, mental health counseling, prescriptions, emergency room, lab testing, diagnostic testing, surgery, hospitalization, etc. The plan covers our students anywhere in the world, every day, no matter whether on campus or on semester breaks.
You must contact the insurance office immediately if you lose your insurance coverage:
All students must review their health insurance policies to determine the date when the student may "age out" of a parent/guardians' policy. It is mandatory that the insurance office be informed as soon as you are aware of this aging-out date, so we may enroll the student in the University plan immediately.
All full-time students at Stony Brook are automatically billed for the University Health Insurance plan at the beginning of each semester.
- Immunization Record Form signed and completed by their physician.
- Documentation of Immunizations on the health form as per NYS law.
- All students must read the medical information about meningococcal meningitis at our website, and complete and return the Meningitis Information Response Form. The information and form can be downloaded . Those who have a SOLAR account and are 18 years of age or older may use SOLAR to submit the response form.
All students are required to pay the SUNY Stony Brook Student Health & Counseling
The fee covers comprehensive health services for both medical and mental health problems, for students and visiting scholars. It is not a substitute for health insurance. The Student Health Service building is the only location on campus where the mandatory health fee can be used. Medical Services that are beyond the scope of the Student Health Service can be obtained either at University Hospital Medical Center or through other medical providers in the community. However, the Student Health & Counseling Fee will not cover the cost of any medical services outside the Student Health Service Building. Call (631) 632-6740 for further information.
