Step 1	Program Announcement	July 5, 2011
	Individual Retirement Session for Employees.	To schedule an appointment call (631) 632-4870
	Interested employees must submit the following: Notice of Irrevocable Intent to Separate from Service form. Title F Leave Application Retirement/resignation letter date of retirement or resignation.	No later than August 30, 2011
	Employee will receive a confirmation notice from HRS. Your Title F Leave must be approved before you will receive your confirmation notice.	Two weeks after application is received.
Step 2	Latest possible resignation or retirement date *Must be off the payroll by August 15, 2012*	No later than August 15, 2012
	Your Title F Leave must be approved in order to participate in the VSP lll program. In the event your Title F Leave is not approved the Irrevocable Intent Separation Application will be automatically canceled.
