Request a Verification
For all verifications of employment, please use the following methods:
- Online:
- Request for Letter of Verification Online Form
- Email:
- Send an email request to HRS_Verify@notes.cc.sunysb.edu
- Fax:
- Requests may be faxed to 631-632-1428, our standard request form may be found here
- In-Person:

You may submit a request for a verification letter at our office locations:
West Campus Office: 390 Administration Building
HSC Office: Level 4, Room 170A, School of Medicine
Hospital employees should contact the University Hospital Human Resources at 631-444-4700.
Long Island State Veterans Home employees should contact the LISVH HR at 631-444-8617.
If you are not a current/previous employee:
Requests may be faxed to 631-632-1428, emailed to HRS_Verify@notes.cc.sunysb.edu or you may mail your requests to:

Please note that all requests must be accompanied by a signed release from the employee authorizing us to release any information or we will not process your request.
Requests will be completed within one business day.
