CPT Employment Letter Template
You may use the template below to guide your employer in preparing an offer letter that meets CPT requirements.
Your employer must print the letter on official company letterhead and include a handwritten or official digital signature. The letter may be provided to you in person or by email.
Replace the bracketed text with your information.

Security Number)
Additional details required by your organization, such as orientation dates, non-disclosure
agreements or pre-employment requirements may be included.
