Financial Aid Forms

Please note that this list does not contain all documents needed or requested by the Financial Aid Office.  Please refer to your SOLAR Account to determine any documents that are required from the university for you to submit in order to determine your financial aid eligibility.

Budget Increase Request Form If you feel that you have exceptional expenses that exceed the cost of attendance estimate you may request that the financial aid office review your budget to assess whether an adjustment is warranted by completing this form is to increase Federal PLUS Loan or Private Loan. Preparatory Coursework Form
A student not enrolled in a degree or certificate program may be eligible for Federal Direct Subsidized/Unsubsidized Loans (and a parent may receive Direct PLUS Loans on behalf of a dependent student) for up to one year if she/he is taking coursework necessary for enrollment in an eligible program.  For additional information on this process, visit the preparatory coursework  section of our Special Circumstances webpage.
Student Loan Change Form This form is to decrease or decline subsidized, unsubsidized, and private educational loans that have previously disbursed. Please note that reducing or cancelling a loan may create a balance due on your account. Visit our Aid and Your Bill  webpage for additional information on understanding how your financial aid is applied toward your charges due.
PLUS Loan Change Form This form is to decrease or decline Graduate PLUS or Parent PLUS loans that have previously disbursed.  Please note that reducing or cancelling a loan may create a balance due on your account.  Visit our Aid and Your Bill  webpage for additional information on understanding how your financial aid is applied toward your charges due.
Permission to Release Information Form All external forms that require information from the Office of Financial Aid & Scholarship Services will only be completed with the student's signature authorizing the release. 
Re-Evaluation Requests If the financial situation of either you and/or your parents (if dependent) has changed significantly, and this change was not reflected on your current FAFSA application, you may be eligible to request a re-evaluation of your financial aid eligibility. A request will only be considered if your current FAFSA reflects an Expected Family Contribution (EFC) greater than 0. This is a review to see if the student is eligible for a   Federal Pell Grant . Petition for Scholarship Reinstatement Students have the option to appeal the loss of their scholarship. We strongly encourage students to wait until they have restored their academic record to the required minimum for scholarship eligibility prior to submitting an appeal.  The request form can be accessed via SOLAR. If you wish to submit an appeal and do have have access to the form, please contact your financial aid office. 

 

For secure and faster processing, students who have outstanding To Do List items are encouraged to utilize the SOLAR Upload feature, when available. Need help scanning your document?
