Course Permission
This form must be completed in order to receive permission to enroll in the following courses: WST: 597, 598, 599, 690, 696, 699, 700, 701, 800, 850. Once your faculty advisor is known a form will be sent to you and your faculty member to fill and sign. Once both signatures are received your permission will be submitted for enrollment. Self Evaluation
This self evaluation is intended for use during your annual review with your advisor and the Graduate Program Director. Speak with your advisor or the GPD for further information.
Advancement to Candidacy
Once you have passed your comprehensive exams, completed all PhD-level coursework, and fulfilled your language requirements, you will use this form to begin the advancement to candidacy process. Before completing the form, please meet with your advisor or the Graduate Program Director to confirm that you meet all eligibility requirements.
Appointment of Dissertation Committee & Authorization to Schedule a Dissertation Date
When you are ready to defend your dissertation, you must complete this form so the department can submit it to the Graduate School for Dissertation Committee approval and authorization of your defense date. Before filling out the form, you should have selected all of your committee members and be prepared to schedule your defense. If you have any questions, please consult your advisor, committee chair, or the Graduate Program Director.
Completion of Degree Requirements
Once your defense has been approved, your signature page has been submitted to the Graduate School, and your dissertation has been uploaded to ProQuest, you must complete this form so the department can forward your degree completion requirements to the Graduate School.
