In order to wear an N95 you must first be medically cleared. See 'respirator forms' drop down below. Once cleared, you may use the following link to schedule a fit test with EH&S. Note that you need to use your @stonybrook.edu email address to log in, not your @stonybrookmedicine.edu address.


  1. Respiratory protection is needed when employees are working in an environment of insufficient oxygen, harmful dusts, fogs, smokes, mists, gases, vapors, and sprays. These hazards may cause cancer, lung impairment, diseases, or death. Email EH&S with any questions.

Any individuals requiring an N95 respirator for their job function first must be medically cleared. Complete the below questionnaire and bring that, along with the certification form, to your medical practitioner. They will then sign off on your certification form.

Individuals who require facial hair for religious or medical reasons may not wear an N95 respirator and therefore do not need to be fit tested. However, a medical questionnaire still needs to be completed and EH&S must still informed of their decision concerning respirator use. In the event of an exemption, the individual would then need to wear a powered air purifying respirator (PAPR) in place of an N95 when it is required. Contact EH&S for more information.
