Blank petition forms may be obtained after speaking with an advisor in the CEAS Undergraduate Student Office, Engineering Building Room 231, Monday - Friday, 10 am - 4 pm.
Students must complete all student sections of the petition and attach a well-written typed personal statement along with appropriate documentation.
A decision for routine requests is normally made within 2 business days; requests for academic reinstatement after suspension and/or dismissal usually require additional time. Decisions will be communicated in writing or in person to the student only.
If a student's petition has been approved, the student is responsible for picking up his/her petition and processing it with the Registrar’s Office, as well as paying any fees.
If a student’s petition is denied, he/she may appeal to the CEAS CASA and Dean, providing additional documentation or support for his/her appeal. Appeals must contain new information to be considered by the CASA and Dean.
