Pharmacology Preliminary Electronic "Short Form" Application (Optional)

We realize that it can be cumbersome and expensive to send the complete application required by the Graduate School. To help prospective applicants determine their overall suitability for our training program prior to submitting their full application, we therefore offer an optional screening service, the Short Form Application. If desired, please fill out the electronic short form application available here and then email the completed form to phmgradadmit@stonybrook.edu.

If your short form application is suitable, we will contact you by December 16, 2022 to recommend that you complete the entire application packet.

Note for International students: We strongly recommend that international applicants use the electronic short form prior to the full application. Please submit the short form by December 10, 2022.
