NASA Disclosure Forms
NASA uses agency-specific Biographical Sketch and Current and Pending (Other) Support forms for grant and cooperative agreement proposals.
Beginning in 2026, NASA implemented NASA-specific disclosure forms within SciENcv and updated the forms to include research security training certifications for covered individuals. Depending on the applicable proposal, investigators may be required to use the NASA-specific SciENcv forms or other forms identified in the solicitation.
Investigators should carefully review each funding opportunity to determine:
- Whether NASA-specific disclosure forms are required.
- Whether SciENcv must be used.
- Whether research security training certification is required.
- Whether additional certifications or disclosure requirements apply.

- Current and pending grants, contracts, and cooperative agreements.
- Research appointments.
- In-kind contributions and research resources.
- Laboratory space, facilities, equipment, or other resources available in support of research activities.

Research Security Training
NASA requires Covered Individuals participating in applicable grant and cooperative agreement proposals to complete research security training before proposal submission.
Covered Individuals must certify that they have completed qualifying research security training within the previous twelve months by signing the certification included in the NASA Biographical Sketch and Current and Pending (Other) Support forms. The applicant institution must also certify compliance as part of the proposal process. These requirements became effective for applicable proposals beginning August 5, 2026.

NASA prohibits Covered Individuals from participating in a Malign Foreign Talent Recruitment Program (MFTRP) where prohibited by federal law and NASA research security requirements.

Changes involving research support, professional appointments, affiliations, outside activities, international collaborations, key personnel, or other reportable matters may require updated institutional disclosures, annual progress report updates, sponsor notification, or prior approval.
Investigators should promptly notify the Office of Sponsored Programs (OSP) and the Office of Research Security (ORS) whenever circumstances change that may affect sponsor disclosures or award requirements.
