Pursuant to DOH orders dated August 18 and August 25, all hospitals and nursing homes must “continuously require all covered personnel to be fully vaccinated against COVID-19, with the first dose for current personnel received by September 27, 2021.” The order broadly defines “covered personnel” as “all persons employed or affiliated with a covered entity, whether paid or unpaid, including but not limited to employees, members of the medical and nursing staff, contract staff, students, and volunteers, who engage in activities such that if they were infected with COVID-19, they could potentially expose patients, residents, or personnel working for such entity to the disease.”
What is required?
This DOH order applies to you, and you must get your first vaccination by September 27, 2021, if you have not already done so. As the order also requires us to ensure compliance, you must show proof of your vaccination status for staffing contingency plans as soon as possible and no later than September 21, 2021.
How to provide proof of my vaccination?

To get vaccinated:
If you haven’t already been vaccinated, we urge you to get your first dose right away as vaccine appointments may be limited here at Stony Brook and across the region and State, as the September 27 state deadline draws closer.
Vaccine PODs for employees will be held at the Stony Brook University Student Activities Center. Click here for a list of dates and times.
Employees may also go to the New York State-run COVID-19 vaccination site conveniently located at Stony Brook University Research and Development (R&D) Park, 1500 Stony Brook Road. The site is open for appointments or walk-ins every Monday through Saturday, from 8 am to 6 pm, and every Sunday, from 8 am to noon.
To check the availability of COVID-19 vaccines throughout the area, visit the New York State “Am I Eligible” website. Walk-ins are welcome, but you can also use the NYS site to schedule an appointment.
Additional information
The order allows for limited medical exemptions with reasonable accommodations, consistent with applicable law. The DOH will be monitoring any such exemptions. The Medical Exemption Form, which outlines exemption criteria and instructions, can be found on ThePulse and the Stronger Together website.
