As a declared Technological Systems Management (TSM) major, please refer to your TSM major checklist to review your progress towards completing your major requirements. This checklist has already been shared with you and can be found in your Google Drive "Shared with me" folder. The title of the spreadsheet is "[Stony Brook ID #] [Last Name] [First Name]".
Reference the "READ ME!" tab on the bottom right for a link to your Program Guide which has your elective and specialization options.
Please review your checklist and add comments to indicate your course plan for the upcoming semesters. (You are not able to make edits to this spreadsheet.) Your comments will be reviewed on a regular basis, please check this spreadsheet frequently to review responses.

Summer and Fall 2026 Academic Advising Online Course Permission Request Forms
To seek permission to enroll in an EST course, please complete the form that corresponds with your request. In order for your request to be considered, all required course prerequisites must be successfully completed. Before submitting your request, you are strongly encouraged to review the Undergraduate Catalog.
For Fall 2026, a number of EST courses have seats reserved for specific populations of students. A course permission request will not be considered if you encounter a “reserve error” upon enrollment.
NOTE: Each of the forms below allows you to request permission for specific EST courses. If the course for which you are seeking permission is not on the corresponding form, you are not permitted to enroll in the course.
Failure to complete the appropriate form will result in your request being automatically denied. Please allow 1-2 weeks for your request to be reviewed. Some requests will not be reviewed until specified dates. See below for details. You will be notified of the decision via email. Please be advised, requests are not guaranteed to be approved and all decisions made by the committee are final.
Enrollment in TSM Major Teaching Assistant, Internship, or Research
Additional paperwork is required to be submitted using the Google Form and requests are subject to review.
Enrollment in EST Courses, Summer 2026
If you are a visiting student, you must attach a copy of your unofficial transcript to verify that you have completed any relevant course prerequisites.
Declared TSM Majors, Fall 2026
You must explain why you are unable to register for the course without permission.
Declared TSM and IND Minors, Fall 2026
To request permission to take an EST course that has a TSM major prerequisite.
These requests are reviewed after all students in the major have been accommodated and notification of the status of this request may therefore be delayed.
All Other CEAS Programs, Fall 2026
If you are planning to declare a TSM or IND major/minor, you may not submit this form until a decision has been made in regards to your major/minor admission.
Non-CEAS Majors, General/Undeclared Students, Fall 2026
Requests are not considered until the start of the semester, pending available space.
