I am an incoming transfer student who was enrolled in an Opportunity Program at my prior institution (EOP, HEOP, or SEEK/CD). How do I apply to the EOP program at Stony Brook University?

Please complete the Health Sciences Transfer EOP Applicant Transfer Verification Form and submit it to Kelly.Bollhofer@stonybrook.edu. *Please note that participation in the EOP program is limited and not guaranteed.

The verification form is due March 1st. Questions regarding EOP can be directed to the above email address.
