Checklist of Grant Proposal Documents for Approval by The Dean's Office

Before you can submit grant proposal materials to the Research Foundation to be checked, they must be approved by the Office of the Dean of the College of Arts and Sciences. Below is a list of the forms and documents that must be included among the materials you take to the dean's office for approval. Everything besides item 6 will be returned to you to deliver to Research Administration the next day. This list is based on an email message that Dean Staros sent to the department chairs about two years ago plus current forms from Research Administration webpages. I believe everything listed is current.
  1. SUSB-299 Form, Proposal/Award Processing Form ("Four Page Form"), signed by the PI, Co-PI's and department chair (http://www.stonybrook.edu/research/forms/ors/susb299.html).
  2. The face page and signature page (if different) of the proposal, signed by the PI and co-PI's.
  3. The proposal abstract or summary.
  4. The proposal budget pages and budget justification.
  5. The facilities and resources statement from the proposal.
  6. The Stony Brook Conflict of Interest Declaration(http://www.stonybrook.edu/research/forms/campus/cid.doc).
  7. Documentation of cost-sharing arrangements, if any.
Items 1 to 6 are mandatory for the dean's office and 7 is required if the "Cost sharing and/or matching funds" box is checked (p. 2, Section V, item 5) on the Four Page Form.

The Dean's office now requires 24 hours to process these papers, and Research Administration requires additional lead time in advance of the target date for grant proposals.
