Replacing Lost, Stolen or Damaged Campus Cards

*** ID Card Replacement Fees: Under normal use and conditions our ID Cards are designed to last at least four years. 

STUDENTS:

If your ID card is lost, damaged, or no longer functions and is less than four years old the $25 replacement fee will apply.  Please note that this fee will apply to cards that do not appear visibly damaged as the Campus Card Office cannot attest to what stresses were placed on the card that caused it to malfunction.  Our ID cards are designed to last at least four years under normal use and conditions.

There will be no charge for a new ID card related to a name change.

ID Card Photo Requirements:  If your photo is older than four years you must submit a new photo or have a new one taken at the Campus Card Office.  If you would like to submit a photo please email campuscard@stonybrook.edu and request a selfie photo link. An email will be sent to you.  Once you receive an email indicating that your photo has been accepted, allow 24 hours before visiting the Campus Card Office.   

Lost Card During Non-Business Hours?
During non-business hours a resident student should report their lost card to the Residence Hall Director (RHD) on call by contacting University Police. The resident student will be issued a temporary access card by the RHD and their old card will have the access feature terminated immediately. Cardholders on meal plans will be able to eat by contacting the Campus Dining Manager at the dining facility and notifying them of the lost card. 


FACULTY and STAFF:

If your ID Card has an expiration date which periodically updates, you will not be charged to replace the card when your new term updates.  However, any replacement of the ID card prior to the expiration will result in a $25 fee.

New Titles/Names/Departments/Etc:  When a staff member switches departments or obtains a new title there will be no charge for a new ID card.  Please note the ID card requirements.

PLEASE NOTE: The Campus Card Office cannot update the information that appears on your ID Card.  Any changes to Name, Title, Certification(s), Credentials, Departments, Expirations, Etc must be handled through your supervisor and Human Resources.  ANY CHANGES MADE BY HUMAN RESOURCES REQUIRE 24 HOURS TO BE REFLECTED ON YOUR ID CARD

Chaperones, Affiliates, and Volunteers

 

Chaperones/Child Protection Badges:

  • Chaperones, Child Protection Badges, Special Event Badges: You must submit a photo to Conference and Event Operations.  You would have received an email from this office requesting a photo as part of your profile setup.  PLEASE NOTE: The Campus Card Office cannot take a photo to be applied to these badges, the photos are submitted only through the Conference and Event Operations Office.   




Volunteers and Affiliates:

  • If your ID Card has an expiration date which periodically updates, you will not be charged to replace the card when your new term updates.  However, any replacement of the ID card prior to the expiration will result in a $25 fee.
  • Incubator members and Contractors will not be charged for their initial ID Card, however any replacement of the ID card prior to the expiration will result in a $25 fee.

  • Replacement ID cards should carry over the access from your previous ID card.  Access is typically refreshed within 60 minutes of the new card being issued.
  • For new ID cards, if you do not have access as anticipated your supervisor will need to email accesscontrol@stonybrook.edu and request access for your card.  Please provide your supervisor with your Stony Brook ID number (9 digits starting with “1” on the front of your card) and the card number (6 digits on the back corner of your card). 
  • All monies deposited on the card for off-campus dining and laundry/vending services are retrievable and will reside on the replacement card.
