Your application and all supporting documentation should be submitted as soon as possible, and no later than Friday, January 3, 2014.
1. Print and complete this application.
2. Return the application, along with a copy of your secondary school transcript (including the latest
quarter’s grades) and the $250 program fee to:

Your application requires signatures of approval from a guidance counselor or principal and from a parent or guardian. Please specify the course number and section preferences on the application. Acceptance is based on the overall quality of your academic record, as well as your preparation for a particular course. Students accepted into the program may take one course. Requests for two courses will be considered for subsequent semesters. If you’re applying for a course with an AP prerequisite, you must include copies of your AP score(s) with your application.
3. Submit the Immunization Form and the Meningitis Response Form (pages 2 and 3 of the application) to Dina Brennan in Admissions.

Students in the Young Scholars Program need to submit a new application for each semester of study at the University and must maintain a GPA of 3.0 or higher to continue in the program. You will be enrolled in the class of your choice on a space available basis after our matriculated students have completed registration for the spring semester. You will be notified of your enrollment by e-mail on or about January 27.
