RE:	Spring 2009 Student Evaluation of Instruction
DATE:	December 5, 2008

Thank you for your continued commitment to the Undergraduate Colleges and the 102 Seminars. The formal student evaluation of instruction and courses should be distributed during the last two weeks of your class.  Instructors should plan accordingly to allow time for your students to complete the evaluations. The surveys should be returned to me no later than May 8, 2008.  If your class meets for a 7 week pattern, please return them to me by March 13, 2008.
Here are a few reminders about the surveys:

1. The scantron forms require the use of a #2 pencil.

2. Select student volunteers to distribute and collect forms in class. 

3. Please step outside of the classroom when students are completing the survey.

4. Your Instructor Number is listed on a label, which is affixed to this folder.

Completed forms are to be placed in a sealed envelope, signed by student volunteers and the faculty member, and delivered to Jason Mastrogiovanni, Melville Library N3071, z-3357.
The processing and all of the protocols regarding return and dissemination of the results will be the same as in the past (Deans, Chairs, Undergraduate Directors and the Undergraduate Student Government).  If you have questions regarding this process, please call me at 2-4378.
