NYSED Graduate Course Verification Form

If it is not clear whether a course that you have taken or would like to take counts toward your certification area, and it does not appear on the approved list of courses, you should submit the NYSED Graduate Course Work Verification Form for signature.
This form is intended ONLY for courses that are not clearly integral to a master’s degree in your content area.

Step 1: Download the form at: http://www.highered.nysed.gov/tcert/pdf/gradcwv.pdf

Step 2: Submit the following to SPD, N221 SBS Building, Stony Brook, NY 11794-4310:

  • A completed NYSED Course Verification Form
  • A copy of your SBU Unofficial Transcript
  • A self-addressed stamped return envelope (SASE). SPD will return an approved form or a denial letter directly to you only if you submit a SASE with your form and transcript.

Step 3: Submit approved form and an official copy of your transcript to NYSED at the address noted on the form. Also, please keep a copy for your records.

 


Statement of Student Responsibility

Students themselves—whether new, returning, or continuing—are responsible for reviewing, understanding, and adhering to their degree and/or certificate program requirements. (SPD’s Non-matriculating [non-degree] Graduate Students [GSP] are responsible for adhering to the guidelines related to non-matriculated status.) All SPD students are responsible for reviewing, understanding, and complying with University and SPD regulations, policies, and procedures, as described in all official publications, the University website, and the SPD website, including, but not limited to, SPD’s online references, the SPD Bulletin and SPD Academic Calendar.
