Frequently Asked Questions about the SBU Health Insurance Plan
HOW CAN I PAY FOR THIS INSURANCE PLAN?
The fee is placed on your University bill. As with all other SBU bills, this is payable via cash, check, money order, credit card; through financial aid for which you may be eligible, or via TOPP (extended payment plan). Payments are made at the Bursar in the Administration Building.
HOW DO I USE THIS INSURANCE PLAN?
You will need to satisfy a per-condition deductible for certain medical services. Therefore, you should first make an appointment with a medical practitioner at the west campus Student Health Service (SHS) for an assessment and possible treatment of your medical problem. By coming to the Student Health Service first, your deductible will be reduced for that medical problem.
WHAT DOCTORS AND OTHER MEDICAL PRACTITIONERS MAY I USE?
You may use any medical practitioners you wish, anywhere in the world. However, medical providers who are part of Aetna Student Health will provide discounts for their medical services, so that your bill will be covered up to the limits of the insurance plan. Usually, you will not need to complete a claim form with these doctors. If you do not use Aetna Student Health providers, you may find that your provider has charged higher than "reasonable and customary" fees, and you may have a balance to pay after the insurance company pays its portion. WHEN WILL I RECEIVE AN ID CARD?
ID cards will be mailed to your home address about 15 days after the waiver period ends. If you need an emergency ID card you may come to the Student Health Insurance Office to pick one up.
WHERE DO I GET CLAIM FORMS?
You should go on-line to www.aetnastudenthealth.com, look for "Help Center," and print out claim forms. These are used if you see a medical provider who is not in the Aetna network. In addition, when you purchase medicines from an outside pharmacy you must always pay for the medicines in full, and then send a claim form with the paid receipt to Aetna Student Health for 100% reimbursement, less a co-payment.

IF I HAVE OTHER INSURANCE ALREADY, HOW FREQUENTLY MUST I FILE A WAIVER?
You must complete a waiver at the beginning of EACH academic year, by the third week of classes. If you are a new incoming student beginning in a fall semester then you must complete a waiver before September 15 and by February 15 if you are a new incoming student in the spring. (the end of the Spring Add/Drop period.) However, if you have any changes in your insurance coverage at any time, you should contact the Student Health Insurance Office for further instructions.
I WAS GIVEN A WAIVER THIS SEMESTER, BUT NEXT SEMESTER I WILL LOSE MY INSURANCE. HOW DO I ENROLL IN THE UNIVERSITY PLAN?

I WILL BE COVERED BY THE UNIVERSITY PLAN IN THE FALL SEMESTER, BUT I WILL HAVE OTHER INSURANCE IN THE SPRING SEMESTER. WHAT DO I DO?
You must complete an online waiver before September 15 in the Fall and by February 15 if you are a new incoming student in the Spring. If you have specific questions, please contact the Student Health Insurance Office.
I AM COVERED BY THE UNIVERSITY PLAN NOW, BUT IN TWO MONTHS I AM GETTING OTHER INSURANCE THROUGH MY SPOUSE’S EMPLOYER. WHAT DO I DO?
You will remain covered by the University plan for the entire semester. However, before the 15th of the following February, you should complete a waiver. If you have any questions, please contact the Student Health Insurance Office.
WHAT IS THE COST OF 2013-14 COVERAGE FOR MY SPOUSE/CHILDREN? WHEN MUST I ENROLL THEM?
Spouse: Fall 2013 semester - $2,451; Spring 2014 semester - $3,421
Child(ren): Fall 2013 semester - $1,238; Spring 2014 semester - $1,728
You must enroll dependents by September 15 for Fall, or February 15 for Spring/Summer. No enrollments can be taken after these dates.
