Policy
Stony Brook University’s Policy at P209 addresses the policy and procedure for SBU investigator financial disclosure and conflict of interest management.Procedure
- All SBU investigators seeking internal or external funding to conduct research, training, or
evaluated testing are required to complete a Conflict of Interest Declaration Form (CID)
with the submission of every proposal.
Access the CID Form here. - Any investigator who checks 'YES' to any question listed on the CID must also complete
an Investigator Disclosure Form (IDF) and submit it directly to SBU’s COI Committee (c/o
Aimee Minton, COI Administrator, Office of the
VP for Research, SBU, 11794-3365) within seven business days of the
sponsor submission deadline.
Access the IDF Form here. - The CID and IDF forms can be completed and attached in COEUS.
Where the Chair and/or Dean have concerns about the answers provided on the CID, s/he will send separate correspondence to the COI Committee (Aimee Minton, COI Administrator, Office of the VP for Research, SBU, 11794-3365) within 7 business days of the sponsor deadline which summarizes those concerns.
