Conflict of Interest
Policy
Stony Brook University’s Policy at P209 addresses the policy and procedure for SBU investigator financial disclosure and conflict of interest management.

Procedure

  1. All SBU investigators seeking internal or external funding to conduct research, training, or evaluated testing are required to complete and sign a Conflict of Interest Declaration Form (CID) with the submission of every proposal.

  2. Any investigator who checks 'YES' to any question listed on the CID must also complete an Investigator Disclosure Form (IDF) and submit it directly to SBU’s COI Committee (c/o Dr. Ann Marie Scheidt, Office of the VP for Research, SBU, 11794-3364) within 7 business days of the sponsor submission deadline.

  3. The CID and IDF forms can be completed, saved electronically and printed. They cannot be electronically submitted.
  4. The completed and signed CID’s for all investigators listed on a proposal should be included with that proposal’s Form 299 (4-page form) and forwarded to the cognizant Chair and Dean for their required signatures prior to submission to the Office of Sponsored Programs.
The process is also summarized as a flowchart.

Incomplete or unsigned CID’s will be returned at this stage to the applicable investigator for corrective action. 
Where the Chair and/or Dean have concerns about the answers provided on the CID, s/he will send separate correspondence to the COI Committee (c/o Dr. Ann Marie Scheidt, Office of the VP for Research, SBU,  11764-3364) within 7 business days of the sponsor deadline which summarizes those concerns.
