American Cancer Society:  The RRC generally has copies of all application materials available in paper copy or on diskette.

American Council of Learned Societies:  Applications available by written request only.  Applicant should state highest degree held and date of receipt; country of citizenship or permanent residence; academic or other position; field of specialization; proposed subject of research; period for which support is requested; and the specific program of interest.  Deadlines vary with program; generally late September through December. American Foundation for AIDS Research:  Complete applications sent only to selected candidates, after submission of a letter of intent with a copy of the Foundation's cover sheet.  Cover sheets are generally available from RRC. American Heart Association:  Deadline for all programs is June 15, annually.  Application materials are available from RRC, and via WWW at: http://www.americanheart.org/presenter.jhtml?identifier=10813

March of Dimes Birth Defects Foundation:  "Applications forms are sent after a 300-word abstract of the proposal has been submitted and considered by us as appropriate."  Deadlines vary with program. See: http://www.modimes.org/Research/index.htm

Muscular Dystrophy Association:  "Applications are not sent to institutions for general distribution.  In order to receive an application, a preliminary letter of intent form must be completed and submitted to MDA for review."  Deadlines:  Research Grants & Fellowships - Dec. 15 and June 15 for Ltrs. of Intent; Jan. 1 and July 1 for completed applications.
