Laboratory Animal Research Forms

Applications for the use of animals in research and educational activities can be downloaded through the links below.

INSTRUCTIONS

The approval form requires the completion of the form History and Risk Assessment Survey for Staff with Animal Contact, for each individual listed in D1 (Personnel) of the Animal Approval Form. After completing this form, print it and submit it to the Center for Occupational & Environmental Medicine.

Download the appropriate forms/appendices to your computer and save them before continuing to fill them in. Most of the MS Word documents are protected forms (checkboxes can be activated with a mouse click and text boxes will expand with user input). You will need a hard copy of the application for purposes of obtaining the Chair's approval.

You are encouraged to return the application as an attachment to an Email message. Note, however, that a hard copy of the application with your signature and that of the Chair will need to be sent separately by regular mail. The completed application should be returned to Mary O'Neill . Questions about the forms and appendices should be directed to Mary O'Neill (631-632-9036). ** Note: A Renewal application can only be used twice per OLAW regulations. A NEW and FULL application must be completed every third year. A PROGRESS REPORT must accompany the new Full application.
