CORIHS Records

The Office of Research Compliance maintains the following CORIHS records:

  1. Current list of CORIHS membership and qualifications;
  2. Minutes of meetings, including information regarding member attendance, discussions held, decisions made, and voting results;
  3. All materials submitted to the committee for initial and continued review of each study, including: CORIHS applications, protocol, submitted and final consent forms, adverse reaction reports, proposed amendments, progress reports, and all correspondence generated between the committee, the investigators, and where applicable, sponsoring agencies.

This information is retained for a period of three years following the inactivation of a project.
