Complete Your Student Health Form

Keeping you safe and healthy is a top priority at Stony Brook. Toward that end, you must have a physician complete and sign your Student Health Form, including documentation of immunizations. This form was included in your Admissions packet and must be mailed to: Student Health Service, Stony Brook University, Stony Brook, NY 11794-3191 PRIOR TO your Orientation day in order to register for classes. Important Note: Failure to submit your Student Health Form will result in deregistration from your fall classes.
