|To:||Research Foundation Employees Eligible for Health Insurance|
|From:||Lisa Coleman, Manager of Employee Benefits|
The annual open enrollment period for the Research Foundation employee benefit plan runs this year from November 1st through November 30th. Research Foundation Central has mailed an October Benefits Bulletin to all employees’ homes, with detailed information on open enrollment for the 2008 plan year. Following are important points for the 2008 plan year.
Please do not forget that the Research Foundation’s health benefit plan offers four premium contribution tiers and the open enrollment period is an opportunity to change your tier:
- Employee only
- Employee and spouse/domestic partner
- Employee and child or children
From November 1st through November 30th all Research Foundation employees eligible for health insurance can make the following health insurance changes that will take effect on January 1, 2008:
- Change from one insurance carrier to another
- Change your covered dependents
- Elect to receive health insurance coverage
- Cancel your health or your dependent health insurance
How to Make a Change
To make one of these changes, you will need to complete a new Research Foundation Benefit Enrollment form. To access this, go to http://www.stonybrook.edu/hr, select Forms & Publications, click on Benefits and select RF Benefits Enrollment Form.
Please contact Cori Ribaudo at (631) 632-6163 or e-mail Cori.Ribaudo@stonybrook.edu for information and forms. Please state your name, department, and zip code and specify what information you would like us to send to you. Please mail or return your form(s) to us no later than November 30th, at the following address:
Human Resource Services/Benefits
Administration Building, Room 390, Z=0751
The co-pay rate for office visits will remain at $20.00. This rate is applicable for the Research Foundation health plans, and they are as follows: Empire PPO, Aetna, Empire Blue Cross HMO, HIP, and HIP/Vytra.
Flexible Spending Accounts
The month of November is a time of open enrollment for the Research Foundation’s Flexible Spending Account (FSA) program. To help save and pay for expenses on a pre-tax basis you may want to enroll in the Health Care Spending Account and/or the Dependent Care Spending Account.
Employees currently enrolled in the FSA program must re-enroll if they wish to continue their contributions in 2008.
For the first time, employees can enroll or re-enroll online by following the step-by-step instructions on the FBMC web site at http://www.FBMC.com or call Cori Ribaudo at (631) 632-6163 for an enrollment packet.
Voluntary Short Term Disability Supplement
You may enroll in the Voluntary Short Term Disability Supplement during the month of November for open enrollment but pre-existing conditions will apply. If you previously enrolled in the program when first eligible, you can increase your coverage level (up to 60 percent of your current salary) without pre-existing condition exclusions. For more information please contact Cori Ribaudo at 632-6163.
Vision Care Plan Improvements
Beginning January 1, 2008 the employee co-payment for any plan approved contact lenses will be $25.00 (some lenses previously had a $45.00 co-payment). Plan selection eyeglasses will continue to be provided with no required co-payment, unless special features are chosen such as hi-index or progressive lenses.
The open enrollment period is a good time to review your life insurance and retirement benefits to keep beneficiary designations current.
You may change your life insurance beneficiary designation at any time by completing a new RF Benefits Enrollment Form and submitting it to the Benefits office.
You may change your beneficiary designation at any time for your retirement plan by contacting the appropriate carrier(s) and requesting the necessary forms.
