Welcome to the Empire Plan's Participating Provider Directory on-line. With the Empire Plan, you can choose a participating provider or non-participating provider for medical services.
These are over 50,000 United Healthcare Empire Plan participating providers. Managed Physical Network (MPN) offers a network of over 5,000 providers for chiropractic care and physical therapy.
This directory lists the Empire Plan's participating providers. The on-line directory is updated weekly. Read your Student Employee Health Plan benefits summary carefully for complete information on your responsbilities. You will need to submit claim forms and pay a higher share of the cost if you choose a non-participating provider or non-network provider.
How This Directory is Organized
You may search this directory two different ways. If you know the zip code you want to search, select "Express (by zip code)". Enter a zip code, desired mileage radius and select the type of provider you want to find and hit the search button. You will receive the results of your search, or suggestions as to how to expand your search, if the search is unsuccessful. Adding the provider's last name, or the first initial of the provider's last name is an optional search.
If you are not sure of your zip code or want to search for providers in a specific city, select "City and State". Enter the state, city, the number and type of provider you want to find and hit the search button. You will receive the results of your search or suggestions as to how to expand your search if the search is unsuccessful.
