New Graduate Student Employee Health Insurance Orientation

WHO: Research Assistants and Fellows

WHEN: September 6, 2007, from:

  • 01:00PM to 02:00PM or
  • 04:00PM to 05:00PM or
  • 07:00PM to 08:00PM

WHERE: SAC Auditorium

All Research Assistants and Fellows MUST bring a COPY of the following items for yourself and each person you wish to cover. Your enrollment application WILL NOT be accepted if the following items are not attached.

  1. Birth Certificate or Passport
  2. Social Security Card

Note: Additional proofs are needed if the student has a spouse or child:

  1. Birth Certificate or Passport
  2. Social Security Card
  3. Marriage Certificate

If a marriage certificate is not available they must supply a copy of the I-20.

The following questions will be answered at the Graduate Student Employee Health Insurance Orientation:

  • Am I eligible for the employee health insurance?
  • Can I enroll my dependents?
  • Is dental and vision offered with this plan?
  • Is prescription covered?
  • Is there an additional dental, vision and prescription plan offered?
  • How much does it cost?
  • What type of doctors can I use?
  • What happens during the school break?
  • What happens if I am an International Student?
  • What if I do not have a Social Security Number or card?
