- Fill out all the requested information on the Establishing an Account Form

- Fill out all of the information that is to be updated on the Account Update Form
. When completed, please fax it to (631) 632-8597 or e-mail us. - Can I place an order for something not carried in the Stockroom?
- A special order can be placed for most items if we currently purchase from a supplier that carries it.
- Can I have supplies delivered?
- We deliver to on-campus sites with a $25.00 minimum order. Fill out all of the requested information on the Delivery Request Form
. When completed, fax it to (631) 632-8597 by 01:00PM and get same day delivery. - Can I place an order for something not carried in the Stockroom?
- Yes, please fill out the Special Order Form. When you have completed the form, please fax it to (631) 632-8597.
